PA Act 153 Clearances

Every athletic trainer working for Go4 in Pennsylvania must have all three Pennsylvania Act 153 clearances on file before their first shift. The steps below walk you through obtaining each one and adding it to your Go4 profile. The process can vary, but we typically see all clearances obtained within two weeks.

HOW TO OBTAIN YOUR THREE CLEARANCES

01  PA Child Abuse History Clearance (Act 151)

Apply at Child Welfare Information Systemhttp://compass.state.pa.us/cwis

  1. Create a CWIS account. Use a computer rather than a phone to avoid form errors.
  2. Complete the application, including every address and household member since 1975.
  3. Submit, then watch for the email notification.
  4. Log back in and download your certificate.

02  PA State Police Criminal Background Check (Act 34)

Apply through PATCHepatch.pa.gov

  1. Select “Submit a New Record Check.”
  2. Enter your personal information.
  3. Submit and download your results.

03  FBI Fingerprint Clearance (Act 114)

Register with IdentoGOhttp://uenroll.identogo.com ·  Service Code 1KG738

  1. Register online or by phone and schedule your fingerprinting appointment.
  2. Pay the fee, roughly $22 to $25 depending on role.
  3. Visit your selected IdentoGO location to be fingerprinted.
  4. Receive your results online if eligible, or by mail.

ADD THEM TO YOUR GO4 PROFILE

1. Save each clearance as a PDF or a clear photo on your device
2. Log in to the Go4 app
3. Open the hamburger menu on the top left
4. Select Credentials > Upload Credentials
5. Select the type of Credential, add the document, and upload


KEEP IN MIND

Submit all three together / Go4 needs the complete set on file before you can be scheduled.

Renew every five years / Each clearance expires 60 months from its issue date.
